List the number and size of vehicles to be used to transport your band so we can plan accordingly:
I have read the entry and accept all rules and agree to abide by same. I agree to assume full liability for any injury or loss to me, my property and any persons associated with my entry, and from any cause whatsoever while participating in the parade. I expressly release the Baraboo Area Chamber of Commerce, and any of their representatives or partners from any and all liability whatsoever, or for any loss or injury.
